Malta has been a delegation of the Sacred Military Order since 1971, and in 1995 Conti John Sant Manduca obtained the honor as delegate/representative for Malta for the Sacred Military Order of Constantine St George. 

His Royal Highness Prince Charles of Bourbon Two Sicilies, Duke of Castro, Head of the Royal House, Grand Master of the Royal Order of St Januarius., Grand Master of the Royal Order of Saint Ferdinand of Merit , Grand Master of the Sacred Military Constantinian Order of Saint George, Grand Master of the Royal Order of Saint George of the Reunion and Grand Master of the Royal Order of Francis I.

H.R.H. the Duke of Castro married in Monaco 31 October 1998 , H.R.h. Princess Camilla, Duchess of Castro Constantinian Dame Grand Cross of Justice , Dame Grand Cross of Honour and Devotion of the Sovereign Military Order of Malta. Their Royal Highnesses currently have two children, HRH Princess Maria Carolina born in Rome in 2003 and HRH Princess Maria Chiara born in Rome 2005.

The Sacred Military Constantinian Order of St. George is run by the Grand Master, helped by the Royal Delegation, formed by the most important people: Grand Prefect, Grand Prior, Grand Inquisitor, Grand Chancellor and Grand Treasurer, the President, the Vice President, the Secretary, and present Deputies. Its present Grand Master, His Royal Highness Prince Ferdinand of Bourbon Two Sicilies, Duke of Castro and Head of the Royal House, is also helped by a Magisterial Juridical Council and by a Counselor for Dynastic Orders and Orders of Knighthood.

The Grand Master is life-administrator of the Order and can promulgate new laws and statutes as well as appoint the highest institutional positions to which he will give the moral, disciplinary and administrative rule of the Order.
The position of Grand Prefect is held by HRH Charles of Bourbon Two Sicilies, Duke di Calabria, heir of the Duke of Castro.

The Knights and Ladies are divided into four Categories: Justice, Special, Grace and Merit. The Order is structured with in many countries of the world including Malta.
Many ecclesiastic personalities are members of the Order and guarantee the pursuit of Christian values and service to the Catholic Church, as established in the Statutes. The most important ecclesiastic representative is the Grand Prior: presently S.E. Cardinal Mario F. Pompedda holds this position.

The seat of the Grand Magistery is at the Grand Master's residence whereas the seat of the Grand Chancellery is in Naples and the Secretariat of the Order is located in Rome, 121 Via Sistina.
